For professionals

A credible private option for families who need practical help, not a care package.

Bridge to Home offers time-limited domestic recovery support for people who are medically discharged but not yet safe managing shopping, laundry, meals and home routines alone — without crossing into regulated personal care.

Referral Fit

Alongside statutory services, not instead of them

Bridge to Home works when reablement has a gap, a wait, or cannot cover the domestic workload families see at home. It is not a replacement for NHS or council pathways where personal care or nursing is required.

  • Private, self-funded domestic and recovery support
  • Suited to Pathway 0 and the gap before formal reablement or when statutory help cannot cover domestic tasks
  • Rapid short-term packages for the first one to three weeks home — often within 24 hours of contact
  • Founded by Lisa with extensive NHS clinical experience — healthcare-led
  • Not CQC-registered: no personal care, medication administration or clinical interventions
  • Written hand-off summary at package completion

Useful referral context

A family may be suitable when the person is medically discharged, can manage without personal care, but needs practical domestic support, shopping, laundry, house preparation and evening welfare calls for one to three weeks.

CQC registration

Bridge to Home does not deliver regulated personal care, so CQC registration is not required for this domestic recovery model. Boundaries are explicit for patient safety and professional reassurance.
